Auto claims checklist

 Names of everyone involved in accident, including witnesses
 Your telephone numbers, including home, work and cell numbers
 A household contact you want handling the claim
 Your policy number
 Date and location of damage
 Description of damage/crash
 Description of injuries to you, your passengers or other people
 Vehicle information, including make, model year and license plate number
 Police department information, including department name, report number and a description of any tickets

Property claims checklist

 Your contact information, including address and telephone numbers
 A household contact you want handling the claim
 Policy type and number
 Date damage occurred
 Driving directions to property
 Description of loss
 Description of damaged property
Location of damaged property

Workers compensation checklist

 Preferred contact of employee, including address, phone numbers and best time to call
 Policy number and billing number
 Date of loss
 Description of loss
 Location
Employee information, including:

  • Name, address and phone number
  • Date of birth
  • Social Security number
  • Weekly salary
  • Marital status and number of dependents

 When, where and how injury occurred
 Description of injury
 Medical provider name and address
 Last day worked
